Missing Beneficiaries

Two of my beneficiaries together with their future dated payments have disappeared off my beneficiaries list. As a result the payments were not made and I have been hit with penalties and late payment charges.

I did not take the beneficiaries off the list, so what has happened to them?

Has anybody else had this experience?

What do I do? Who do I speak to?
