Proof of residence

Could someone please assist with a form on which I can complete the details confirming proof of residence of my reletive residing in my home.

I can get the same from on ABSA, Nedbank & FNBs sites, but the new form Standardbank requires, are not available on the side (00194615 2017-06), however a copy of the older version is.

Would Standardbank accept an older version form if I complete and email that to the bank...?
